A comprehensive diabetes solution powered by Populance to improve outcomes
TROY, Mich. Oct. 13, 2025 - Health Alliance Plan (HAP) today announced the launch of the Diabetes Care Connection (DCC), a personalized diabetes management program powered by Populance, a leading provider of coordinated, physician-supported care. The program combines clinical guidance, digital tools and personal coaching to help members better manage their diabetes, improve blood sugar control and prevent complications. The initiative will now be available to all HAP members, meeting individuals wherever they are in their diabetes journey. This marks a major step forward in HAP’s mission to improve members' health through a holistic, personalized approach to healthcare.
More than 28,000 HAP members live with diabetes. The new partnership with Populance builds on proven results from the Henry Ford Health DCC program, which has significantly lowered blood sugar levels for most participants in the pilot program. Nearly 80 percent of enrolled patients reduced their A1C by an average of 1.6 percent, with improvements sustained over 18 months.
“Lowering A1C isn’t just about numbers, it’s about protecting the heart, kidneys, nerves and eyes, while helping people live longer, healthier lives,” said Margaret Anderson, president of HAP. “We’re taking a broader view across the risk continuum, not only focusing on high-risk members but also supporting those just beginning their diabetes journey.”
The program expansion allows HAP to reach members outside of the Henry Ford Health system, broadening access and impact. It targets not just high-risk individuals but also moderate- and low-risk members, those with prediabetes, and individuals not yet undergoing regular A1C testing.

The enhanced program offers a comprehensive suite of interventions, including:
- Diabetes self-management training and education: American Diabetes Association-recognized education focused on seven key self-management behaviors.
- Empowerment groups: Peer support communities to foster learning and shared experiences.
- Medical nutrition therapy: Personalized nutrition planning in collaboration with registered dietitians.
- Diabetes in active control: Intensive coaching and continuous glucose monitoring to improve adherence and outcomes.
- Diabetes in pregnancy support: Specialized diabetes educators to support pregnant members managing diabetes.
- Medication therapy management: Integrated medication support to improve adherence and effectiveness.
