Flex+ Rewards

Beginning January 2026, Healthy Living Rewards and flex benefits will be on one card, the HAP Flex+ Rewards prepaid Visa®. Your new card should have arrived in the mail in December.

Make sure to activate your card. You can do that, check your card balance and view all of the ways you can spend your funds on the HAP member portal.

With the Healthy Living Rewards program, you can earn big rewards for doing simple things like:

  • Completing your Annual Wellness Visit
  • Maintaining healthy blood pressure
  • Completing your A1c test
  • Completing your diabetes eye exam
  • Completing a recommended colon screening
  • Completing your breast cancer screening
  • Getting your flu vaccine
  • Learning about prescription home delivery
  • Connecting with your Navigator

Once you finish the activity, your reward goes right onto your prepaid Visa® card. Then, you can use your rewards card online or in-store at most major retailers that accept Visa® prepaid cards, including many of your favorites like Costco, Kroger, Meijer, Walgreens, Walmart and more. Use your rewards to pay for groceries, household essentials and even dining out. Just keep in mind, you can’t use rewards to buy alcohol, tobacco, firearms or explosives and your card may not be usable at locations associated with these items.

If you were enrolled in the Healthy Living Rewards program for 2025, you may still receive rewards you earned in 2025 through April of 2026. A voucher from InComm Incentives will be mailed to you. You can redeem your rewards online or by mail for your choice of a gift card from national retailers.

Remember, while you’ll only have one card, your flex benefit is separate. It gives you a set amount to spend on certain items. Your 2025 prepaid benefits Mastercard will no longer work after December 31, 2025.
