Healthy Living Reward Program

Coming in 2026: Flex+ Rewards

Your Healthy Living Rewards and flex benefits are on one card with the HAP Flex+ Rewards prepaid Visa® card. If your plan does not have a flex benefit, you will still receive a Healthy Living Rewards card after you consent to the program. If you’ve used this program in the past, remember rewards may vary from year to year.

You can earn big rewards this year for doing simple, healthy things like completing your Annual Wellness Visit, completing recommended screenings and learning more about prescription home delivery. For a full list of rewards, log into your member account.

Once you finish your activity, your reward goes right onto your prepaid Visa® card.

Then, you can use your rewards at many of your favorite stores like Costco, Kroger, Meijer, Walgreens, Walmart and more. Use your rewards to pay for groceries, household essentials and even dining out. You can use your card online or in-store at most major retailers that accept Visa® prepaid cards. Just keep in mind: you can’t use rewards to buy alcohol, tobacco, firearms or explosives and your card may not be usable at locations associated with these items.

Remember, while you’ll only have one card, your flex benefit is separate. Your flex benefit gives you a set amount to spend on certain items.

You’ll be able to request a card, activate your card, check your card balance and view all the ways you can spend your funds on the HAP Member Portal or the HAP app.

  • View your balance, review transactions and search nearby retailers

Closing out your 2025 Healthy Living Rewards

If you were enrolled in Healthy Living Rewards program for 2025, you may still receive a voucher for your 2025 rewards through April, 2026. InComm Incentives will mail the voucher to you and you have one year to redeem it before it expires. You can redeem your rewards online or by mail for your choice of a gift card from national retailers; the gift cards do not have an expiration date.

2025 Healthy Living Reward Program

 

 

It's easy to enroll and start earning rewards. Simply:

  1. Complete the form on this page.
  2. Complete eligible tests and screenings. Tests and screenings must be performed on the person who has registered, but all eligible members of a household can participate. Complete between Jan. 1 and Dec. 31, 2025 to qualify for a reward. View eligible tests and screenings below.
  3. Get rewarded automatically. We'll track your rewards by looking at the claims your HAP doctor sends us. You may only receive one reward per type of test each year.
  4. Receive your rewards. InComm InCentives will mail you a voucher for the value of the rewards you earned.
  5. Redeem your rewards online or by mail for your choice of a gift card from national retailers.

*Your Annual Wellness Visit is different from your Annual Physical. It should be noted that they can both be completed during the same visit. Below are some key differences:

Annual Wellness Visit

  • Reviews your medical and family history
  • Make a schedule for preventive screenings
  • Tests for cognitive wellness
  • Create or update a list of medical providers and prescription drugs 

Annual Physical

  • Assess risk factors for preventable diseases
  • Perform lung, head and neck, abdominal and neurological exams
  • Take urine and blood samples and submit for lab testing.
