Diabetes management that meets members where they are
HAP continues to invest in members with or at high-risk of diabetes, including the recent launch of our new Diabetes Care Connection (DCC) program. The DCC is a focused disease management service which is open to everyone, but especially helpful for those with complicating conditions such as heart or kidney disease.
The program has been a success so far with the average member achieving a 10%-20% decrease in A1C levels after 6 months, which is an equivalent of about 1.6 A1C points per patient. And the wins aren’t temporary, that number has proven to be consistent, with improvements maintained over at least an 18-month period.
“A reduction of 1.6 points is a big deal,” said Dr. Charles Bloom, chief medical officer at HAP. “For someone starting with an A1C of 8.5%, that’s the difference between uncontrolled diabetes and being in a healthy, managed range. For a patient at 7%, a drop of that size often moves them out of the diabetes range altogether.”

HAP and Henry Ford Health have teamed up to provide world-class and cost-effective care with the new Henry Ford Health Select plan. Enrolled members get access to renowned Henry Ford Health facilities and providers and others within the high-performing HMO network. By staying in this network, we have seen a reduction in group premiums of 15 - 20%.
Henry Ford Health Select’s High-Performing Network includes 13 Henry Ford Health acute care hospitals, more than 550 care sites and 9,400 affiliated providers. HAP and Henry Ford Health chose this network of doctors, hospitals and specialists because they consistently deliver high-quality care at lower costs for our members.
Lowering pharmacy costs
Prescription drugs are a huge driver of health care costs. Keeping these high prices in mind, HAP thoughtfully selected the medication on our formulary to maximize savings while still providing your employees with the medication they need.
