How HAP Helps You Manage Chronic Conditions: What to Know
Living with chronic conditions such as diabetes, kidney disease or heart disease can be overwhelming. Managing prescriptions, seeing multiple doctors, scheduling appointments and keeping track of test results can feel like a full-time job. HAP is here to support you on your journey.
HAP offers a free Care Management Program to support members with chronic conditions. The program helps make care easier, more coordinated and more personalized.
What is chronic care management?
Chronic care management focuses on improving your overall health and quality of life. It also aims to reduce stress and remove barriers to getting care. HAP’s program includes assessment, planning, care coordination, education and advocacy.
The goals are to improve patient safety, ensure high-quality care and support long-term health in a cost-effective way.
How the program works
When you join the program, you are matched with a case manager who stays in touch with you by phone. This person will work with your doctor to create a care plan that fits your needs. Your case manager helps guide you through different care settings, such as moving from the hospital back home.
They also connect you with local services, mental health specialists and other resources to support both your physical and emotional health. Your case manager tracks your progress and helps you stay focused on your health goals.
This service is free for members. You, your doctor or your caregiver can refer you to the program.
Support for managing diabetes
If you are living with diabetes, HAP offers Diabetes Care Management to help you manage either type 1 or type 2 diabetes. The program is personalized and offers tools to support your daily routine.
Key steps in diabetes self-care include:
- Know your A-B-Cs. Work with your healthcare team to track your A1C, blood pressure and cholesterol.
- Adopt healthy habits. Follow a balanced meal plan, stay active and avoid tobacco.
- Take medications as directed. Meds help control your blood sugar and supports your overall health.
- Monitor your glucose. Check your levels regularly and learn the signs of high blood sugar, including fatigue, frequent urination and blurred vision.
- Schedule regular checkups. See your primary care provider, dentist and eye doctor, and stay up to date on recommended vaccines.
This approach gives you practical tools and expert support to help you take charge of your health.
