The plan is available to both small and large employer groups in seven Michigan counties: Wayne, Oakland, Macomb, Lapeer, Genesee, Jackson and Washtenaw. It is also open to individuals and families through the Health Insurance Marketplace via HAP CareSource.

By combining HAP’s coverage expertise with Henry Ford Health’s renowned care network, this plan offers something rare: coverage that is truly integrated with care delivery.

The model is proven. Earlier in 2024, HAP and Henry Ford launched a Medicare Advantage plan that now serves more than 4,000 members. The pilot program for this plan showed that bringing together healthcare services and insurance coverage can lead to better health results. Members in HAP’s Henry Ford Medicare Advantage plan scored 6 percent higher on key Medicare quality ratings than members in HAP’s other Medicare plans.

• Access to Henry Ford Health and more than 9,400 affiliated providers
• Over 550 care sites and 13 acute care hospitals
• Coordinated coverage designed for better outcomes
• Primary care provider assignment and referrals to manage costs and guide care
• Ancillary services including labs, imaging, therapy, urgent care and behavioral health through HAP’s HMO network

The plan is offered as both a Health Maintenance Organization and an Exclusive Provider Arrangement, giving members access to a network chosen for its ability to deliver high quality care at lower costs.

Looking Ahead

Quoting for employer groups begins in fall 2025, with coverage starting January 1, 2026.
